Does the fourth player HAVE to play as Nabbit in multiplayer, or is there an option for a Red Toad or something as well? Because man, fuck Nabbit. I want my friends to know they are shithouse, not give them a free ride because of it.

Doesn't look like it. If there are four players playing, one of them has to play as Nabbit and be allowed to troll everyone else.
To add to the insult, anything Nabbit collects will be tallied and converted to 1UPs at the end of the level. So either kick the fourth player away, or endure Nabbit.

This really is New Super Nabbit U. You can play as Nabbit (without all those dying) by simply holding ZL when entering a level.
