Speaking of Metroids, the deeper areas of the Phazon Mines scare the bejeezus out of me in Metroid Prime. The music is really creepy, and the fact that the version in MP Trilogy has a drastic increase in the amount of Fission Metroids in that area doesn't help any. >_< It's less frightening when I have the Phazon suit, because then I don't have to worry about falling into the raw Phazon while dealing with enemies, but that still doesn't keep the area from being scary shit.

Floormasters/Wallmasters . . . The Redead's as well, with there strange body grabbing and paralysing scream. The Phazon Metroids in Prime 3 are scary, you see them travelling about through the see through floor and the they phaze through the walls . . . And Spy's in TF2 can be scary sometimes . . .

I just remembered one that nobody has mentioned so far. It was probably the first time I was terrified in a game, and that fear never really left me in later replays:

Man how could I've forgotten about that... THING. I remember picking up the key only to immidiatly drop it and wait 2-3 minutes before going for the keyhole. The longer you waited, the longer it took him to get back to you. I made sure there was no way I'd see him again. Remembered even more stupid fears from some rather obscure games, so I linked some to videos: Hokuto no Ken for NES: So you're this Jet Li wannabe that has to beat up random grunts by kicking or punching. Kicking sends 'em flying, punching has them oink like pigs while quickly inflating and deflating only to blow up in little pieces after awhile... I made sure to never punch anything in that game. Burger Time for NES: This picture says a lot more then I could. The idea of food coming to life and trying to gnaw on you instead freaked me out back then. Plus I didn't know how the game worked so all I could do was run away when I ran out of salt. Karateka for NES: Other then the creepy fighting music, the part where you're running forward creeped me out because I didn't know when to expect another enemy running into me. When I won I'd just walk a few steps then wait for the enemy to show up so I could hear him closing in on me. Bird Week: Some weird bird simulation game for Famicom, pretty fun. You're supposed to catch butterflies and feed 'em to your two hungry chicks every stage. The baby birds will flap around hysterically when they're ready to be fed. But if you're too slow the baby birds will eventually just sit there and slowly sink in until a creepy music starts playing as a ghostly chick flies towards the skies. Binary Land: Only because of that darn arachnophobia of mine. Basically you control 2 penguins and must move both through a maze to get 'em back together, but the maze is littered with giant killer spiders and their webs that entangle your penguin. Being trapped in a spider infested tomb is pretty much my worst nightmare. Sim Copter for PC: I loved this glitch-fest. You fly around in a helicopter and help out in town: extinguish fire, chase robbers, call in ambulance etc. Sometimes you'd have to pick up wounded civilians and deliver 'em to the hospital. These civilians would constaly make these choking barf noises and if you were too slow their bandaged face slowly became a bloody fucked up mess. By that point I'd just eject 'em from my helicopter into a lake.

A lot of things in 32/64 bit games. The bad graphics for some reason make scary monsters even SCARIER. It's something I now appreciate but it wasn't good for a timid youth back then, and it still isn't! I mainly point to the Zelda games, Sin & Punishment and Perfect Dark. Also the Resident Evil games.
For more recent games, Metroid Prime the first time when you expected nothing. Same with Metal Gear Solid 2.

I'm scared shitless of Rockrocs. You know, the stone alligators in DKC's "Stop & Go Station." It's like "Oh, a rock. That's a pretty deco--HOLY SQUIRREL BALLS IT MOVED!"

That Stop N Go station was probably the first and only time I found the secret warp to the end of the stage, just through fear of those damn Rockrocs. It was a long time before I cleared that stage properly.

Aquas in Star Fox 64. You're underwater in a sub and you can't see most of everything unless you fire a torpedo, and even then there's still not a lot of light. And I've got a fear of drowning, which doesn't help at all. >_< I've mostly gotten over it though, thankfully.

The giant, fire-breathing dogs in Megaman 2, and by extension the giant, yarn-spitting cats in Megaman 3. I'm over them, though they scared me when I was a wee TEi. XD

Oh yeah, this isn't really Nintendo related, but the Yeti from Skii (it was a PC game that IIRC came with Windows back then) used to scare me. The thing came out of nowhere, man.
